Doctors: Make it easy for your patients’ parents to keep track of their immunization and growth records. These immunization and growth record folders include a growth chart, immunization schedule and immunization record chart.

Each 6” x 9-7/8” folder has one inside pocket and is imprinted in black ink. The growth chart is printed on the inside flap, and the immunization record is printed on the outside of the pocket. A one-time $15 setup charge will apply if you would like to have the outside of the folders personalized with your practice’s name and contact information.

Folders will be made in your choice of 11 different paper stocks: white marble crush, bright white felt, white smooth, white fiber, natural, cream, gray, ice blue, moonstone, rose and sage.
