These pressure sensitive labels are designed for patients to sign in advance, for use when sending items to insurance companies that will not accept signature on file. Designed to be compatible with our 2006 revision ADA forms.

4-1/4” x 1-1/16” labels are printed 2 wide in red ink on 8-1/2” x 4-7/8” sheets, with 6 labels on each sheet.
